    Requesting a New Program or Updating a Program

    Information to Provide to SIS Analyst

    Program Information Needed:

    1. Description - Name of the Program as well as description to appear on student transcripts.

    2. Degree - Title of the Degree (if applicable).

    3. Credits - Total number of Credits and Hours for program.

    4. Will the program be FT/PT? How many week and months is the program expected to run?

    5. SAP Table - Default is minimum 2.0 GPA, specify if needed to be different.

    6. CIP code.

    7. Campuses - What campuses will the program be offered in?

    8. Provide program curriculum as it will appear in catalog.

    Course Information Needed:

    1. Code - New course codes, see point of contact for assignment.

    2. Course Title - The title will appear on all academic records, max 45 characters.

    3. Course Type - ex. Lecture/Lab, Clinical, Comp.

    4. Hours/Credits - This is set up at the course level and cannot be different by program.

    5. Prerequisites/Corequisites - Course codes needed. Prerequisites and corequisites are set up at the course level and cannot be different by program.

    6. Billing Information - Tuition rate for courses.

    7. Is the course an Elective or a Core Course?

    General Information Needed:

    List any courses that may be allowed as substitutes that fall outside of the program.

    What is the Start Date (term)?

    To do by Registrar's Department once Program is ready:

    Schedule courses in the Schedule of Classes.

    Lead Time:

    Program requests should be submitted with a 12 week lead time, prior to the next term registration start. Please submit requests via ticketing system. Spreadsheets available for New Program Request and New Course Request to assist with information needed.
