The State of Illinois requires students and personnel to be vaccinated against COVID-19.
Who is required to be vaccinated?
By September 19th, 2021, students and personnel must either attest that they have been fully vaccinated, or declare an submit weekly negative COVID tests: Vaccination Verification Form
On Aug. 26, 2021, Gov. Pritzker issued an executive order that all higher education students and employees that are on ground must be fully vaccinated against COVID-19 vaccination as a condition of participating in on ground classes or employment, effective September 5th, 2021. PCHS is updating its policies and systems to comply with this mandate.
Exemptions
Exemptions may be sought for documented medical conditions and sincerely held religious beliefs.

Additionally, Gov. Pritzker’s proclamation mandating vaccines for state employees, health care workers, and higher education students and personnel requires documentation to be provided when seeking a medical exemption or an exemption for a sincerely held religious belief. The College is updating its policies and systems to comply with this mandate.
Weekly Negative Test Submissions
Students, faculty or staff who are on campus for class or clinic shifts and elect not to be vaccinated must complete weekly negative COVID test results using the online test submission form.

Deadlines
Personnel must submit the COVID-19 Vaccine Verification Form indicating they are fully vaccinated or declaring an exemption:
- As soon as possible after being fully vaccinated or have decided to declare an exemption no later than September 19th, 2021, whichever date is earliest.
- Newly hired personnel are required to submit the form upon hire that they are fully vaccinated or declaring an exemption. If they are not yet fully vaccinated, the form is completed as soon as possible after being fully vaccinated.
Privacy
Vaccination verification information is private and confidential:
- Managers may not use, share or disclose this information for any purpose other than upholding compliance with state regulations.
- Employees may not ask colleagues about their vaccination status.
- Instructors may not ask their students about their vaccination status, nor will they have access to students’ records or be expected to confirm students’ vaccination status.

I’m fully vaccinated. What do I need to do to show proof to the college?
Please scan or take a photo of your COVID-19 vaccination record and upload it using this form onto our secure site.
I am a fully online student/remote employee of the College, do I have to follow the same guidelines?
If you are 100% remote, you do not need to follow the guidelines at this time. Please note that you are not permitted to come to the college for any reason other than for getting treatments in the college clinic. Should you need to come to visit the college for any reason outside of a clinic treatment, you must first show proof of vaccination or a negative Covid test.

How do I submit my weekly test results?
Please take a scan or a picture of your weekly negative test results and upload them using this form.
How do I show documentation that I have a medical or religious exemption?
Step 1: Fill out the PCHS exemption form which can be found here.
Step 2: Once you complete the exemption form, you must then submit the completed form online here.
Please keep in mind that those who receive an exemption, still must get weekly negative covid tests to come to the college each week.
What do I do if I receive a positive Covid test?
Please do not come to campus. Please complete the COVID Diagnosis form and await further instructions.
When does this all take effect?
The mandate goes into effect on Sunday September 19th.
Does this impact patients of the clinic or visitors of the College?
No. This order does not apply to patients of the clinic or visitors of the college (including vendors and candidates seeking employment).
